Where to access the best Transperineal prostate biopsy

Nearly all NHS Trusts and Private Hospitals and their Urological Surgeons now provide Transperineal biopsies. Historically these were ‘saturation’ biopsies, where many cores were taken. Increasingly these have been replaced by targeted biopsies where many fewer samples are taken. The ultimate goal in the future is to only require biopsies of the index lesion, that is the lesion responsible for cancer spread.

We have had reports from leading Consultant Urologists that a lot of mpMR scans and the reports are of poor quality even though PROMIS complaint resulting in inaccurate biopsy results as samples had been taken from the wrong regions of the prostate. If you only spend time and maybe money on one thing, get the best mpMR scan reported by the best Uro Radiologists.

If ‘cognitive’ fusion is used, that is looking at the MRI images and then at the ‘live’ ultrasound of the prostate to decide where to put the needles, then the best Urologists performing this will be those that offer focal therapy. The reason being that they are very experienced in accurately reading MRI and locating lesions to be sampled on ultrasound.

This is of course equally true of Radiologists who also perform biopsy. Oliver Hulson in Leeds being an example.

Remember you perfectly entitled to ask to be referred to a Consultant/hospital that does offer this service.

Finally, the PRECISION study which is currently active, is comparing accuracy and performance of fusion biopsies compared to cognitive biopsy. It is anticipated that fusion biopsy, where MRI images are ‘elastically’ overlaid onto the live ultrasound images, with prove more accurate. It should be noted that the PI AI system for lesion detection from MR scans, can output to fusion systems.
